1. The premium is calculated when the policy is purchased and is based on age, the number of insured persons included in the policy and payment method. Representative example calculated for an individual (1 insured person between the ages of 0 and 24) who takes out a MyBox Salud policy with a monthly premium on 01/09/2022. In the above case the premium would be €59 per month, with taxes and surcharges included, for the first 3 years. This premium is valid until the policy is renewed, when it will be updated in accordance with the policy conditions.

2. Promotion valid exclusively for new policies purchased by individuals between 20-06-2023 and 31-12-2023. Customers can obtain a reimbursement of two months of the MyBox Salud insurance premium in the first year, and one month in the second year, provided the policy is taken out during the specified promotion period. Minimum contract period 36 months. Promotion can be used in conjunction with other campaigns or promotions. 3. Maximum discount applicable to the MyBox Salud insurance premium per insured person/month and until renewal, for contracts with five or more insured persons made before 31-12-2023.

4. The initial duration of the MyBox Salud policy is three years and premiums are payable monthly. No surcharges will be applied for splitting the payment. If the insurance policy is renewed, the renewal premium may be recalculated in accordance with the conditions of the policy. From the fourth year, the MyBox insurance policy will be renewed automatically every year. Subject to the conditions of MyBox. 5. Adeslas Salud y Bienestar is a service accessible to persons insured with Adeslas who are over 18 years old. This service and the digital health services are not available in Asturias, Navarre or the Basque Country. Check the health insurance terms for Asturias, Navarre and the Basque Country.
